Automation for users requiring access to shared folders

We want to define an automated approval workflow to handle the File Server share access requests. i.e. Users can click on the shared folder for which they require access to and the file server should provide the user with a shared folder request form. The form will have multiple fields i.e. business justification, Line manager & IT person or helpdesk email ID etc. Once user fills all the required details the post that the workflow should trigger an email to user line manager and respective IT for further processing?

any idea how can i achieve the same?
