In Place Upgrade issues

Hello all,

I have done an in place upgrade from windows 2012 R2 to windows 2016, after a few hours the upgrade goes well.

Two days later the server didn't have windows 2016, seams it rolled back to windows 2012 R2 and i don't understand how this is possible. This is the second time the server is back with windows 2012 R2.

Can any one please advise me on this?
