Hi. I am getting a very specific freeze with a dual monitor setup.
First, my setup:
Nvidia GTX 1080.
Primary LG 27GN950 monitor. 4K 120Hz resolution set in Nvidia control panel. Connected over Displayport.
Secondary LG TV, 1080p, connected over HDMI.
The freeze happens like this - dual monitors work fine in this configuration, until I turn the primary monitor off. This causes the display to shift to the HDMI monitor, which works fine. The issue is when I turn the DP monitor back on. This causes a freeze, with the image frozen rendered on the HDMI monitor, but a no signal report on the DP monitor.
When checking Reliability History later, I get the following readouts:

I can't locate a minidump report either, since it seems to not generate on a freeze - unless I'm not doing something right?
System works fine when the DP monitor is in 4k 95Hz mode. I have tried multiple fixes, including full GPU driver wipes and rollbacks, Windows rollbacks, reseating components, and DP cable replacement.
It'd be great if I could get some help with this.
