Windows Media Player CD Ripping Questions

1. Why is it that most CDs don't have album art when viewing in windows media player and upon ripping them to device? Is this an issue with the CD or WMP?*Sometimes this will happen with both of the two images, or with one and not the other.2. How do I make it so that ripped CDs appear in a file without it automatically making a file for the Artist in which it is contained? e.g. I just want it to make a file called "[album name]", not "[artist]" and inside that "[album name]"
