how to issue certificates based on the content of certificate request

In a past thread, an answer to a question was to issue certificates based on
the content of the certificate request,
rather than using certificate templates.

Can someone explain how this works?

This would require that the certificate be submitted to either a standalone
CA or to a 3rd party CA. If you are using an enterprise CA, then you are
limited to requesting that the request be based on a specified certificate
template. Now you can provide the subject of the certificate in the request
(if the template allows this), and some settings can over-ride certificate
template settings (going with a longer key, shorter duration period,
different CSP), but not extension changes such as adding an application
policy
