How do I stop my pc from periodically exiting and re-entering fullscreen while gaming or sometimes exiting full screen all together?

While playing nearly any game on my pc, seemingly at random my pc will exit full screen and go black for a few seconds and then load back into the game, or it will load into the Windows home screen.
