Windows 10 fails to lock screen automatically.

I have Windows 10 Pro on a laptop running in the power-on configuration. See below for details on system & settings. I have it set to sleep and turn off the screen, and lock on screen saver, but the machine remains unlocked and often won't even sleep when left unattended.
