How can I remove pc from my link to windows mobile application?

How can I remove an account from link to windows mobile application. I'm facing an issue I reinstalled windows on my pc and my link to windows app is still showing me that pc. I connected my pc to link to windows app again. And know I see to same named pc on my mobile application. How can I remove old one from mobile application?
