How to transfer Microsoft lumia app data from Microsoft lumia hpone to another Microsoft lumia phone?

I want to transfer my App data to a Microsoft lumia phone from my old Microsoft lumia phone, I habe tried Backup from settings and SD card swapping, however those methods did not work.
