windows vista is stuck in update mode, rebooting loop!

Starts for a few minutes then shuts down to only restart the process again. Nothing will stop it! can any one help? I don't have a recovery disk Have run diagnostics and it says it is all fine. I ran it in the onic mode and here it says that media

has failed ?
