standard employee IT policy

I work for a small business. My boss has asked me to come up with a an IT

policy for new employees to sign.

 

Could someone direct me to where I might be able to download a standard

boilerplate business IT policy? Basically it just needs to be something that

new employees sign that states that company info is private and proprietary,

email and internet usage, etc. etc.
